Special Sponsor's Gift Form:
Use this form to make a gift to your family over and above your regular gift. This is not a Seeds of Love gift.
Seeds of Love Form:
Use this form to make a donation as a birthday or other gift for friend or family member or as a memorial gift. A card will be sent to the person in whose name the donation is made or to their family.

Use this form to make a donation to any of our programs.
